We understand the challenges that individuals who identify as transgender, or somewhere along the gender non-conforming spectrum, have in finding appropriate medical care to meet their individual needs. We have a dedicated Transgender Services Navigator who can help you connect with providers and services, including:

Understanding and affirming the lives of trans and gender diverse persons

Middlesex Health offers multiple support groups for trans and gender diverse members of our community and their allies. These groups are free and are facilitated by medical staff with special expertise in transgender care.

Support groups are held online. You are welcome to join one or more of the groups that is/are the best fit for you.

GENDER DYSPHORIA

Not all transgender persons have gender dysphoria.

Transgender is an umbrella term that encompasses many identities (e.g., male-to-female, female-to male, cross dresser, cisgender, gender fluid), all of which challenge conventional/traditional gender expectations. Many persons who are transgender do not experience distress/discomfort related to their identity/experience. For transgender persons who do experience distress/discomfort, gender dysphoria may be present.

Gender dysphoria is complex.

Among adults, gender dysphoria is related to a strong and persistent belief that there is an incongruence between their experienced/expressed gender and their sex characteristics, and/or that they have the typical feelings/reactions of the other gender. These beliefs are accompanied by strong and persistent desires as well. These desires can include: wanting to be the other gender, wanting to be treated as the other gender by others, and wanting to have sex characteristics of the other gender rather than one’s own. (Due to developmental differences, children and adolescents experience gender dysphoria differently than adults.)

Gender transition is a process of affirmation.

There are three primary paths of gender transition. For some, the exploration and pursuit of one path is desirable. Whereas for others, two or more paths is the preferred choice.

The journey on each path, to include experienced rewards and challenges, is unique to each person:

  • Psychological transition: Involves experiences such as exploring beliefs about gender in general, examining one’s own gender identity, and/or processing loss related to gender transition
  • Social transition: Involves experiences such as coming out, presenting as one’s identified gender, and/or changing legal documents to be consistent with one’s gender identity
  • Medical transition : Involves experiences such as hormone therapy, hair removal, speech therapy, and/or gender affirming surgeries

Your health care provider might make a diagnosis of gender dysphoria based on:

  • Behavioral health evaluation. Your provider will evaluate you to confirm the presence of gender dysphoria and document how prejudice and discrimination due to your gender identity (minority stress factors) impact your mental health. Your provider will also ask about the degree of support you have from family, chosen family and peers.
  • DSM-5. Your mental health professional may use the criteria for gender dysphoria listed in the Diagnostic and Statistical Manual of Mental Disorders (DSM-5), published by the American Psychiatric Association.

Gender dysphoria is different from simply not conforming to stereotypical gender role behavior. It involves feelings of distress due to a strong, pervasive desire to be another gender.

Some adolescents might express their feelings of gender dysphoria to their parents or a health care provider. Others might instead show symptoms of a mood disorder, anxiety or depression. Or they might experience social or academic problems.

Treatment can help people who have gender dysphoria explore their gender identity and find the gender role that feels comfortable for them, easing distress. However, treatment should be individualized. What might help one person might not help another.

Treatment options might include changes in gender expression and role, hormone therapy, surgery, and behavioral therapy.

If you have gender dysphoria, seek help from a doctor who has expertise in the care of gender-diverse people.

When coming up with a treatment plan, your provider will screen you for mental health concerns that might need to be addressed, such as depression or anxiety. Failing to treat these concerns can make it more difficult to explore your gender identity and ease gender dysphoria.

Changes in gender expression and role

This might involve living part time or full time in another gender role that is consistent with your gender identity.

Medical treatment

Medical treatment of gender dysphoria might include:

  • Hormone therapy, such as feminizing hormone therapy or masculinizing hormone therapy
  • Surgery, such as feminizing surgery or masculinizing surgery to change the chest, external genitalia, internal genitalia, facial features and body contour

Some people use hormone therapy to seek maximum feminization or masculinization. Others might find relief from gender dysphoria by using hormones to minimize secondary sex characteristics, such as breasts and facial hair.

Treatments are based on your goals and an evaluation of the risks and benefits of medication use. Treatments may also be based on the presence of any other conditions and consideration of your social and economic issues. Many people also find that surgery is necessary to relieve their gender dysphoria.

The World Professional Association for Transgender Health provides the following criteria for hormonal and surgical treatment of gender dysphoria:

  • Persistent, well-documented gender dysphoria.
  • Capacity to make a fully informed decision and consent to treatment.
  • Legal age in a person's country or, if younger, following the standard of care for children and adolescents.
  • If significant medical or mental concerns are present, they must be reasonably well controlled.

Additional criteria apply to some surgical procedures.

A pre-treatment medical evaluation is done by a doctor with experience and expertise in transgender care before hormonal and surgical treatment of gender dysphoria. This can help rule out or address medical conditions that might affect these treatments This evaluation may include:

  • A personal and family medical history
  • A physical exam
  • Assessment of the need for age- and sex-appropriate screenings
  • Identification and management of tobacco use and drug and alcohol misuse
  • Testing for HIV and other sexually transmitted infections, along with treatment, if necessary
  • Assessment of desire for fertility preservation and referral as needed for sperm, egg, embryo or ovarian tissue cryopreservation

Behavioral health treatment

This treatment aims to improve your psychological well-being, quality of life and self-fulfillment. Behavioral therapy isn't intended to alter your gender identity. Instead, therapy can help you explore gender concerns and find ways to lessen gender dysphoria.

The goal of behavioral health treatment is to help you feel comfortable with how you express your gender identity, enabling success in relationships, education and work. Therapy can also address any other mental health concerns.

Therapy might include individual, couples, family and group counseling to help you:

  • Explore and integrate your gender identity
  • Accept yourself
  • Address the mental and emotional impacts of the stress that results from experiencing prejudice and discrimination because of your gender identity (minority stress)
  • Build a support network
  • Develop a plan to address social and legal issues related to your transition and coming out to loved ones, friends, colleagues and other close contacts
  • Become comfortable expressing your gender identity
  • Explore healthy sexuality in the context of gender transition
  • Make decisions about your medical treatment options
  • Increase your well-being and quality of life

Therapy might be helpful during many stages of your life.

A behavioral health evaluation may not be required before receiving hormonal and surgical treatment of gender dysphoria, but it can play an important role when making decisions about treatment options. This evaluation might assess:

  • Gender identity and dysphoria
  • Impact of gender identity in work, school, home and social environments, including issues related to discrimination, abuse and minority stress
  • Mood or other mental health concerns
  • Risk-taking behaviors and self-harm
  • Substance misuse
  • Sexual health concerns
  • Social support from family, friends and peers — a protective factor against developing depression, suicidal thoughts, suicide attempts, anxiety or high-risk behaviors
  • Goals, risks and expectations of treatment and trajectory of care

Other ways to ease gender dysphoria might include use of:

  • Peer support groups
  • Voice and communication therapy to develop vocal characteristics matching your experienced or expressed gender

Gender affirming care for transgender and gender non-conforming people.

Transgender and gender diverse patients can receive a full range of medical services through our Gender Affirming Care program. We offer hormone therapy, referrals to surgical affirmation and basic post-op care. Our model of care includes behavioral health consultants for brief mental health visits, second surgical letters and expert psychiatric care. We offer all primary care services including STI/HIV testing, Pre-Exposure Prophylaxis for HIV (PrEP), Post-Exposure Prophylaxis for HIV (PEP) and care for people living with HIV through our Ryan White Program.

We offer patient-centered primary care in addition to gender affirming services, including pertinent and necessary health screenings with cultural and gender sensitive providers.

Do you use the informed consent model for transgender hormone therapy, how old do i have to be to receive transgender hormone therapy at community health care.

Per Community Health Care policy, we can prescribe hormones to those 18 years or older. Anyone younger can be seen for primary care and referred to a specialist.

How do you decide how to prescribe hormone therapy to me?

We ask a lot of medical and personal questions, most of which we ask everyone, some are specific to hormone therapy. These are important for learning how to keep your whole body healthy. We also must make sure you are able to provide informed consent, which means you are able to understand and make major health decisions for yourself. We are committed to supporting people through an essential and exciting transition. We also take seriously our responsibility to do this safely so you can make your transition as smooth, easy, and enjoyable as possible.

How long will it take until you write my first prescription?

This depends. Most commonly we will write a prescription on the third visit. If we identify health risks we may need to take time to get you healthy and stable enough to go through this life-changing process safely and comfortably. We know for many people it is easier to make healthy decisions in a body that better matches who they really are. We believe in shared informed decision-making. This means we try to make decisions with you, not for you.

So what happens at the first two visits?

At the first one we will get a health history and order baseline labs. At the second one we will talk about your gender story and your personal goals for transition.

How will I be treated at your clinic?

The mission statement of Community Health Care is “to provide the highest quality health care with compassionate and accessible service for all”. We are committed to treating all of our patients with the dignity they deserve. All staff should use your correct pronoun. Our electronic medical record, like most, is not well designed to capture gender diversity. However we do document preferred names and pronouns and expect ourselves to be aware of this information. If you are misgendered please feel free to politely remind staff of your identity. If you have any concerns let your provider or a manager know. Your experience is important to us.

How do I get started?

Call the main appointment line at (253) 722-2161. You may choose to disclose your gender status to be scheduled, possibly more quickly, in a specialty hormone therapy appointment. Alternatively you may just ask to schedule with your chosen provider and they can help with system navigation after you meet.

I wish my PCP could do this so I don’t have to meet new people, can you help them keep taking care of me?

Hormone therapy to feminize or masculinize the body

Talk to your PCP before starting testosterone or estrogen therapy. They’ll talk to you about any risks or side effects. And they'll help make sure that you’re taking the right amount of masculine or feminine hormones. Alcohol and tobacco use raise the risk of side effects when taking hormones. Ask your provider how much alcohol is safe for you and about ways to quit smoking, especially if you are considering surgery. 

Treatments to delay the onset of puberty

For parents of children who would like to explore gender-affirming care, please contact their pediatrician, who can talk to you about your options.

Surgery to change primary sexual characteristics (genital and breast surgery)

For top surgery, one letter* of support is required. For bottom surgery, one letter* of support is required, as well as a letter* from Endocrinology, if applicable. See the Member Coverage Booklet for details. Additionally, you can contact Specialty Services .

While you’re getting ready for your surgery, it’s also important to stay in shape — that means eating healthy and exercising regularly.

Taking care of your health after surgery

After surgery, talk to your PCP about safe sex and being screened for sexually transmitted diseases. You’re also covered for preventive health services based on your birth anatomy, including screenings for breast, cervical, and prostate cancers.

Gender Support Services

These may include:

  • Facial changes to make you look more feminine or masculine 
  • Decreasing the size of your Adam’s apple 
  • Chest implants 
  • Voice modification surgery 
  • Electrolysis (except when it’s a part of genital reconstruction) 
  • Hair grafts or implants

Your mental health is important to us, and you might seek the assistance of a mental health professional for a number of reasons throughout your gender identity journey. Your mental health provider will evaluate your readiness for the next steps in your treatment and make recommendations.

We are committed to creating a welcoming environment for everyone. We recognize that many of our members use a chosen first name rather than their legal name.

How to Share Your Chosen First Name with Us (Note that MyChart uses the term “ preferred ” name.)

  • During e-check under “Details about me” or at the kiosk.
  • When you check in at the front desk or schedule an appointment with the Call Center.

What Else You Can Expect

  • We will make every effort to use your chosen first name during your visit with us. (A legal name is sometimes required for things like medical claims and lab work.)

* A note about letter requirements : A letter of support is required for each surgical procedure a patient wishes to undergo, and must be specific in nature with identifying the surgical procedure and indicating that the outcome of gender-affirming surgical intervention has been assessed, with risks and benefits being discussed with the patient.  The letter must be timely in relation to the requested surgical procedure and will be considered appropriate when dated within six months of a given procedure.

Note: All single occupancy bathrooms at Yale Health are designated as All Gender. These bathrooms are available in all clinical departments.

Transgender Hormone Therapy

Transgender men and women desire a permanent identity as a member of the gender with which they identify. This transition usually involves hormonal therapy and may also involve surgery.

We offer hormonal therapy for transgender individuals. People seeking transgender surgery are referred to well-established health care facilities that specialize in this procedure.

Hormonal Therapy

Transgender hormonal therapy (also called hormonal reassignment) allows development of secondary sexual characteristics that reflect the person's preferred gender identity.

Hormonal therapy for transgender people replaces the hormones naturally occurring in their bodies with those of the other sex.

  • Woman-to-man hormonal treatment (testosterone treatment) is provided to promote male characteristics, such as a male pattern of body hair, a male voice and a male physical shape. The treatment also results in stopping monthly menstruation.
  • Man-to-woman hormonal treatment is provided to inhibit male characteristics and promote feminine characteristics, such as breast formation and elimination of body hair growth.

Both male hormones (androgens) and female hormones (estrogens) are present in men and women alike, but in vastly different amounts. Giving the right amount of the right hormone is important in the life-long management of transgender hormonal therapy.

Psychological Evaluation

To obtain sex reassignment therapy, a full psychological or psychiatric evaluation by a certified professional will be performed prior to the start of hormonal therapy. This is important to ensure that the diagnosis of transgender re-assignment is not clouded by an underlying psychiatric disorder. 

A local transgender program called Pathways  offers professional and supportive programs dealing with issues of gender identity. Pathways also offers psychiatric evaluations to program participants, as well as information, guidance about resources and peer group support.

Therapy Based on Guidelines

Initial care includes a physical exam and blood test to check the individual’s overall health. Hormonal therapy is geared to an individual’s goals and therapy is monitored by clinical response and blood tests. Therapies are adjusted as needed based on response. Hormones are given as oral or injected medications.

Therapy is based on care standard guidelines established by The Endocrine Society called Endocrine Treatment of Transgender Persons. The guidelines were developed by a task force of content experts and co-sponsored by the European Society of Endocrinology, European Society of Pediatric Endocrinology, Lawson Wilkins Pediatric Endocrine Society, and the World Professional Association for Transgender Health. The guidelines address treatment of adolescents, hormonal therapy for transgender adults, long-term care and sex re-assignment surgery.

People who desire transgender hormonal therapy are encouraged to see a trained medical professional, who will prescribe the right medications and monitor your care over time. The physician carefully watches for complications as you slowly begin taking an increased level of hormones.

For people with  gender dysphoria,  transitioning from one’s birth gender to their desired gender can be a big step. It can involve hormonal treatment as well as surgery. Some professionals recommend a mental health screening and psychotherapy beforehand as part of the process. But is that really necessary?

The AMA Journal of Ethics discussed the debate in a recent commentary.

On one hand, counseling can help patients better understand the complex procedures and the adjustments that will be needed, even if one has been living as their desired gender for a while. Also, many view gender reassignment surgery as permanent, and patients need to be prepared.

On the other hand, many patients are confident about their decision and see no reason for a mental health professional to intervene or approve the transition. Others feel that the transition can be modified if they change their mind, even if a complete reversal is not possible.

According to the commentary, the World Professional Association for Transgender Health (WPATH) advises mental health screenings and recommends psychotherapy before any body modifications are made. However, such decisions could be made on a case-by-case basis.

It is still important for patients to provide informed consent before any hormonal or surgical procedure is conducted, acknowledging that they understand the procedures, the risks and benefits, consequences, and alternatives, the commentary noted.

Before scheduling an initial appointment with the Mary Bridge Children’s Gender Health Clinic, you will need a referral from your primary care provider. This referral should be sent to the Mary Bridge Children’s Pediatric Endocrinology Clinic with a gender dysphoria diagnosis.
