Preparation is the single most important part of making a successful presentation. It is an absolutely crucial foundation, and you should dedicate as much time to it as possible, avoiding short-cuts. Good preparation will ensure that you have thought carefully about the messages that you want (or need) to communicate in your presentation and it will also help boost your confidence.

There are a number of aspects that you need to consider when preparing a presentation. They include the aim of the presentation, the subject matter, the audience, the venue or place, the time of day, and the length of the talk. All these will affect what you say and how you say it, as well as the visual aids that you use to get your point across.

The Objective

Whenever you are asked to give a presentation or speak to a group of people, you need to start by asking the purpose of the presentation.

In other words, what is the presentation expected to achieve, and what outcome(s) do the organisers and the audience expect?

These outcomes will shape your presentation, because it must be designed to achieve the objective and deliver the desired outcomes.

For example, you might be asked to give a talk to a gardening club. You might be told that the purpose of the talk is to fill a regular meeting slot, and that the members of the club have expressed a desire to learn more about pruning. You therefore know that your talk needs to be entertaining, fairly light, but knowledgeable, and that your audience wants to learn something new.

As you prepare your presentation, make sure you keep asking yourself:

“How is saying this going to help to achieve the objective and outcomes?”

The Subject

The subject of your presentation or talk about comes from the objective. They are linked, but they are not necessarily exactly the same thing.

For example:

The subject may be given to you by the organisation that has invited you (such as talking about pruning to the gardening club).

You may be knowledgeable in a particular field (perhaps you have an interest in local history).

The subject may be entirely your choice within certain limitations (you might, for example, be asked to give a presentation at an interview on a project which you feel has particularly developed your skills).

The Audience

Before preparing material for a presentation, it is worth considering your prospective audience.

Tailoring your talk to the audience is important and the following points should be considered:

The size of the group or audience expected.

The age range - a talk aimed at retired people will be quite different from one aimed at teenagers.

Gender - will the audience be predominantly male or female?

Is it a captive audience or will they be there out of interest?

Will you be speaking in their work or leisure time?

Do they know something about your subject already or will it be totally new to them?  Is the subject part of their work?

Are you there to inform, teach, stimulate, or provoke?

Can you use humour and, if so, what would be considered appropriate? If you are in any doubt about this, it is probably best to avoid anything even remotely risqué.

It is important to have as much advance information as possible about the place where you are going to speak.

It can be helpful to arrange to see the venue before the event. It does much to quell fear if you can visualise the place while you are preparing your talk. However, even if you cannot visit, you will probably find it helpful to know:

The size of the room;

The seating arrangements (for example, theatre-style, with rows of seats; or round-table);

The availability of equipment, e.g., microphone, laptop and projector, flip chart;

The availability of power points and if an extension lead is required for any equipment you intend to use;

If the room has curtains or blinds. This is relevant if you intend to use visual aids, and so that you can ensure the correct ambiance for your presentation;

The position of the light switches.  Check if you need someone to help if you are using audio/visual equipment and need to turn off the lights;

The likelihood of outside distractions, e.g., noise from another room; and

The availability of parking facilities so you do not have a long walk carrying any equipment you might need to take.

If this information is not available ahead of time, it will help to get there a bit early, to give you time to set up.

There will often be no flexibility in the time of day that a presentation is made. However, it does affect what you can do, and how you might organise your presentation, because of the likely state of your audience (see box).

How time of day can affect your audience

The morning is the best time to speak because people are generally at their most alert. However, as it gets towards lunch time, people begin to feel hungry and lose concentration. This is particularly true if the event has not included a coffee break.

After lunch, people often feel sleepy and lethargic. If you are given a slot immediately after lunch, it is a good idea to get your audience involved. A discussion or getting your audience moving about will work a lot better than simply presenting a lot of slides. A flip chart may also be a more useful tool than a laptop and projector, especially if it means you can open blinds and use natural light.

Towards the end of the afternoon, people again tend to lose concentration as they start to worry about getting home, the traffic or collecting children from school.

Evening or Weekend:

Outside regular office hours, people are more likely to be present because they want to be rather than because they have to be there.  There is a better chance of audience attention in the evening. However, if the presentation goes on for too long, people may have to leave before you have finished. People will also be less tolerant of a poor presentation because you are in their time, not their employer’s.

Length of Talk

Always find out how long you have to talk and check if this includes or excludes time for questions.

Find out if there are other speakers and, if so, where you are placed in the running order.  Never elect to go last.  Beware of over-running, as this could be disastrous if there are other speakers following you.

It is important to remember that people find it difficult to maintain concentration for long periods of time. This is a good reason for making a presentation succinct, well-structured and interesting. Aim for 45 minutes as a maximum single-session presentation, and preferably leave at least 10 or 15 minutes for questions. Nobody minds finishing a session early.

Providing Information in Advance

Always check what information you will need to provide in advance.

Organisers of big events and conferences often like to have all the PowerPoint presentations several days ahead of the event. This gives them time to load all the presentations, and make sure that they are properly branded for the event.

Some events also need speakers’ biographies ahead of time, to put in conference literature. When you are asked to give the presentation, make sure you ask what is needed by when—and then supply it.

You will not be popular if you turn up on the day and announce that you have completely rewritten your presentation on the train. It is entirely possible that the organisers may even not be able to accommodate that, for example if the audio-visual is being supplied by a separate company or by the venue.

And finally…

Being asked to give a presentation is an honour, not a chore.

You are representing your organisation or yourself, if you are self-employed. You are also not there by right, but by invitation. It is therefore important that you put in the time and effort to ensure that you deliver what your audience wants. That way, you may just be invited back another time.

For many people the thought of delivering a presentation is a daunting task and brings about a  great deal of nerves . However, if you take some time to understand how effective presentations are structured and then apply this structure to your own presentation, you’ll appear much more confident and relaxed.

Here is our complete guide for structuring your presentation, with examples at the end of the article to demonstrate these points.

Why is structuring a presentation so important?

If you’ve ever sat through a great presentation, you’ll have left feeling either inspired or informed on a given topic. This isn’t because the speaker was the most knowledgeable or motivating person in the world. Instead, it’s because they know how to structure presentations – they have crafted their message in a logical and simple way that has allowed the audience can keep up with them and take away key messages.

Research has supported this, with studies showing that audiences retain structured information  40% more accurately  than unstructured information.

In fact, not only is structuring a presentation important for the benefit of the audience’s understanding, it’s also important for you as the speaker. A good structure helps you remain calm, stay on topic, and avoid any awkward silences.

What will affect your presentation structure?

Generally speaking, there is a natural flow that any decent presentation will follow which we will go into shortly. However, you should be aware that all presentation structures will be different in their own unique way and this will be due to a number of factors, including:

  • Whether you need to deliver any demonstrations
  • How  knowledgeable the audience  already is on the given subject
  • How much interaction you want from the audience
  • Any time constraints there are for your talk
  • What setting you are in
  • Your ability to use any kinds of visual assistance

Before choosing the presentation’s structure answer these questions first:

  • What is your presentation’s aim?
  • Who are the audience?
  • What are the main points your audience should remember afterwards?

When reading the points below, think critically about what things may cause your presentation structure to be slightly different. You can add in certain elements and add more focus to certain moments if that works better for your speech.

What is the typical presentation structure?

This is the usual flow of a presentation, which covers all the vital sections and is a good starting point for yours. It allows your audience to easily follow along and sets out a solid structure you can add your content to.

1. Greet the audience and introduce yourself

Before you start delivering your talk, introduce yourself to the audience and clarify who you are and your relevant expertise. This does not need to be long or incredibly detailed, but will help build an immediate relationship between you and the audience. It gives you the chance to briefly clarify your expertise and why you are worth listening to. This will help establish your ethos so the audience will trust you more and think you’re credible.

2. Introduction

In the introduction you need to explain the subject and purpose of your presentation whilst gaining the audience’s interest and confidence. It’s sometimes helpful to think of your introduction as funnel-shaped to help filter down your topic:

  • Introduce your general topic
  • Explain your topic area
  • State the issues/challenges in this area you will be exploring
  • State your presentation’s purpose – this is the basis of your presentation so ensure that you provide a statement explaining how the topic will be treated, for example, “I will argue that…” or maybe you will “compare”, “analyse”, “evaluate”, “describe” etc.
  • Provide a statement of what you’re hoping the outcome of the presentation will be, for example, “I’m hoping this will be provide you with…”
  • Show a preview of the organisation of your presentation

In this section also explain:

  • The length of the talk.
  • Signal whether you want audience interaction – some presenters prefer the audience to ask questions throughout whereas others allocate a specific section for this.
  • If it applies, inform the audience whether to take notes or whether you will be providing handouts.

The way you structure your introduction can depend on the amount of time you have been given to present: a  sales pitch  may consist of a quick presentation so you may begin with your conclusion and then provide the evidence. Conversely, a speaker presenting their idea for change in the world would be better suited to start with the evidence and then conclude what this means for the audience.

Keep in mind that the main aim of the introduction is to grab the audience’s attention and connect with them.

3. The main body of your talk

The main body of your talk needs to meet the promises you made in the introduction. Depending on the nature of your presentation, clearly segment the different topics you will be discussing, and then work your way through them one at a time – it’s important for everything to be organised logically for the audience to fully understand. There are many different ways to organise your main points, such as, by priority, theme, chronologically etc.

  • Main points should be addressed one by one with supporting evidence and examples.
  • Before moving on to the next point you should provide a mini-summary.
  • Links should be clearly stated between ideas and you must make it clear when you’re moving onto the next point.
  • Allow time for people to take relevant notes and stick to the topics you have prepared beforehand rather than straying too far off topic.

When planning your presentation write a list of main points you want to make and ask yourself “What I am telling the audience? What should they understand from this?” refining your answers this way will help you produce clear messages.

4. Conclusion

In presentations the conclusion is frequently underdeveloped and lacks purpose which is a shame as it’s the best place to reinforce your messages. Typically, your presentation has a specific goal – that could be to convert a number of the audience members into customers, lead to a certain number of enquiries to make people knowledgeable on specific key points, or to motivate them towards a shared goal.

Regardless of what that goal is, be sure to summarise your main points and their implications. This clarifies the overall purpose of your talk and reinforces your reason for being there.

Follow these steps:

  • Signal that it’s nearly the end of your presentation, for example, “As we wrap up/as we wind down the talk…”
  • Restate the topic and purpose of your presentation – “In this speech I wanted to compare…”
  • Summarise the main points, including their implications and conclusions
  • Indicate what is next/a call to action/a thought-provoking takeaway
  • Move on to the last section

5. Thank the audience and invite questions

Conclude your talk by thanking the audience for their time and invite them to  ask any questions  they may have. As mentioned earlier, personal circumstances will affect the structure of your presentation.

Many presenters prefer to make the Q&A session the key part of their talk and try to speed through the main body of the presentation. This is totally fine, but it is still best to focus on delivering some sort of initial presentation to set the tone and topics for discussion in the Q&A.

The above was a description of a basic presentation, here are some more specific presentation layouts:

Demonstration

Use the demonstration structure when you have something useful to show. This is usually used when you want to show how a product works. Steve Jobs frequently used this technique in his presentations.

  • Explain why the product is valuable.
  • Describe why the product is necessary.
  • Explain what problems it can solve for the audience.
  • Demonstrate the product  to support what you’ve been saying.
  • Make suggestions of other things it can do to make the audience curious.

Problem-solution

This structure is particularly useful in persuading the audience.

  • Briefly frame the issue.
  • Go into the issue in detail showing why it ‘s such a problem. Use logos and pathos for this – the logical and emotional appeals.
  • Provide the solution and explain why this would also help the audience.
  • Call to action – something you want the audience to do which is straightforward and pertinent to the solution.

Storytelling

As well as incorporating  stories in your presentation , you can organise your whole presentation as a story. There are lots of different type of story structures you can use – a popular choice is the monomyth – the hero’s journey. In a monomyth, a hero goes on a difficult journey or takes on a challenge – they move from the familiar into the unknown. After facing obstacles and ultimately succeeding the hero returns home, transformed and with newfound wisdom.

Another popular choice for using a story to structure your presentation is in media ras (in the middle of thing). In this type of story you launch right into the action by providing a snippet/teaser of what’s happening and then you start explaining the events that led to that event. This is engaging because you’re starting your story at the most exciting part which will make the audience curious – they’ll want to know how you got there.

Remaining method

The remaining method structure is good for situations where you’re presenting your perspective on a controversial topic which has split people’s opinions.

  • Go into the issue in detail showing why it’s such a problem – use logos and pathos.
  • Rebut your opponents’ solutions  – explain why their solutions could be useful because the audience will see this as fair and will therefore think you’re trustworthy, and then explain why you think these solutions are not valid.
  • After you’ve presented all the alternatives provide your solution, the remaining solution. This is very persuasive because it looks like the winning idea, especially with the audience believing that you’re fair and trustworthy.

Transitions

When delivering presentations it’s important for your words and ideas to flow so your audience can understand how everything links together and why it’s all relevant. This can be done  using speech transitions  which are words and phrases that allow you to smoothly move from one point to another so that your speech flows and your presentation is unified.

Transitions can be one word, a phrase or a full sentence – there are many different forms, here are some examples:

Moving from the introduction to the first point

Signify to the audience that you will now begin discussing the first main point:

  • Now that you’re aware of the overview, let’s begin with…
  • First, let’s begin with…
  • I will first cover…
  • My first point covers…
  • To get started, let’s look at…

Shifting between similar points

Move from one point to a similar one:

  • In the same way…
  • Likewise…
  • Equally…
  • This is similar to…
  • Similarly…

Internal summaries

Internal summarising consists of summarising before moving on to the next point. You must inform the audience:

  • What part of the presentation you covered – “In the first part of this speech we’ve covered…”
  • What the key points were – “Precisely how…”
  • How this links in with the overall presentation – “So that’s the context…”
  • What you’re moving on to – “Now I’d like to move on to the second part of presentation which looks at…”

Physical movement

You can move your body and your standing location when you transition to another point. The audience find it easier to follow your presentation and movement will increase their interest.

A common technique for incorporating movement into your presentation is to:

  • Start your introduction by standing in the centre of the stage.
  • For your first point you stand on the left side of the stage.
  • You discuss your second point from the centre again.
  • You stand on the right side of the stage for your third point.
  • The conclusion occurs in the centre.

Key slides for your presentation

Slides are a useful tool for most presentations: they can greatly assist in the delivery of your message and help the audience follow along with what you are saying. Key slides include:

  • An intro slide outlining your ideas
  • A  summary slide  with core points to remember
  • High quality image slides to supplement what you are saying

There are some presenters who choose not to use slides at all, though this is more of a rarity. Slides can be a powerful tool if used properly, but the problem is that many fail to do just that. Here are some golden rules to follow when using slides in a presentation:

  • Don’t over fill them  – your slides are there to assist your speech, rather than be the focal point. They should have as little information as possible, to avoid distracting people from your talk.
  • A picture says a thousand words  – instead of filling a slide with text, instead, focus on one or two images or diagrams to help support and explain the point you are discussing at that time.
  • Make them readable  – depending on the size of your audience, some may not be able to see small text or images, so make everything large enough to fill the space.
  • Don’t rush through slides  – give the audience enough time to digest each slide.

Guy Kawasaki, an entrepreneur and author, suggests that slideshows should follow a  10-20-30 rule :

  • There should be a maximum of 10 slides – people rarely remember more than one concept afterwards so there’s no point overwhelming them with unnecessary information.
  • The presentation should last no longer than 20 minutes as this will leave time for questions and discussion.
  • The font size should be a minimum of 30pt because the audience reads faster than you talk so less information on the slides means that there is less chance of the audience being distracted.

Group Presentations

Group presentations are structured in the same way as presentations with one speaker but usually require more rehearsal and practices.  Clean transitioning between speakers  is very important in producing a presentation that flows well. One way of doing this consists of:

  • Briefly recap on what you covered in your section: “So that was a brief introduction on what health anxiety is and how it can affect somebody”
  • Introduce the next speaker in the team and explain what they will discuss: “Now Elnaz will talk about the prevalence of health anxiety.”
  • Then end by looking at the next speaker, gesturing towards them and saying their name: “Elnaz”.
  • The next speaker should acknowledge this with a quick: “Thank you Joe.”

From this example you can see how the different sections of the presentations link which makes it easier for the audience to follow and remain engaged.

How Google Works – by Eric Schmidt

This presentation by ex-Google CEO  Eric Schmidt  demonstrates some of the most important lessons he and his team have learnt with regards to working with some of the most talented individuals they hired. The simplistic yet cohesive style of all of the slides is something to be appreciated. They are relatively straightforward, yet add power and clarity to the narrative of the presentation.

Since being released in 2009, this presentation has been viewed almost four million times all around the world. The message itself is very powerful, however, it’s not an idea that hasn’t been heard before. What makes this presentation so powerful is the simple message he is getting across, and the straightforward and understandable manner in which he delivers it. Also note that he doesn’t use any slides, just a whiteboard where he creates a simple diagram of his opinion.

The Wisdom of a Third Grade Dropout – by Rick Rigsby

Here’s an example of a presentation given by a relatively unknown individual looking to inspire the next generation of graduates. Rick’s presentation is unique in many ways compared to the two above. Notably, he uses no visual prompts and includes a great deal of humour.

However, what is similar is the structure he uses. He first introduces his message that the wisest man he knew was a third-grade dropout. He then proceeds to deliver his main body of argument, and in the end, concludes with his message. This powerful speech keeps the viewer engaged throughout, through a mixture of heart-warming sentiment, powerful life advice and engaging humour.

As you can see from the examples above, and as it has been expressed throughout, a great presentation structure means analysing the core message of your presentation. Decide on a key message you want to impart the audience with, and then craft an engaging way of delivering it.

By preparing a solid structure, and  practising your talk  beforehand, you can walk into the presentation with confidence and deliver a meaningful message to an interested audience.

It’s important for a presentation to be well-structured so it can have the most impact on your audience. An unstructured presentation can be difficult to follow and even frustrating to listen to. What makes a great presentation?

Before you get started, it pays to know what makes an excellent presentation.

1. It informs your audience by providing reliable information.

People want to be informed. They want to learn something new. For this reason, you should look for reputable links. The information should be as recent as possible, and at least less than a year old.

Your research work doesn’t need to be from online sources. You could also cite printed sources from the library. Double-check all of your sources and make sure they have substantial research and statistics to back them up.

2. It persuades your audience to take action.

A presentation should be persuasive. This is especially true for business presentations and product demos. You might also want to appeal to your readers through emotions.

3. It educates your audience and empowers them to make informed decisions.

Presentations are, by nature, educational. You might be introducing your audience to a new idea, product, or service.

2. Create an outline.

For example, graphs and charts are often used to highlight comparisons in data. You can also use them to inform your audience of a specific data point.

It’s worth noting that a poorly-designed graph or chart could ruin your presentation if proven false or shabbily done. Make sure that your data are correct, and your diagrams or charts are correctly labeled. Don’t just use pie charts because they look hip and smart. 10. Show, don’t tell.

Stories are a powerful medium to get your audience to sit up and listen to you. For this reason, aim to “show” rather than “tell” your audience about a topic, insight, or idea.

For example, don’t just state facts or figures about the dangers of not investing in their retirement. Instead, share the story of someone you know who failed to plan for their retirement, nudging your audience towards making their own conclusions or insights.

Don’t bombard your audience with too much information all at once. Avoid jargon or complex concepts without sharing a story that’ll resonate with them. With compelling storytelling, you can create anticipation and then slowly build up to your key points.

Bullet points are great tools to emphasize tips, features, or steps in lists. However, it’s best to avoid them in presentations because they don’t help your audience retain information.

Research even supports this recommendation. In 2014, the International Journal of Business Communication published the results of their research —   The Use of Visualization in the Communication of Business Strategies: An Experimental Evaluation .

The researchers wanted to learn whether the use of visuals is superior to text (a bulleted list to be specific) in communicating the strategy of the financial services branch of an international car manufacturer.

The researchers concluded the following:

“Subjects who were exposed to a graphic representation of the strategy paid significantly more attention to, agreed more with, and better recalled the strategy than did subjects who saw a (textually identical) bulleted list version.”

Instead of using bullet points, consider using icons or visuals.

13. Choose fonts that are easier to read.

The quality of your font could affect your audience’s reaction to your presentation. Don’t just use the first standard font that pops up in your presentation editor.

Your font should match the mood and intent of your presentation. If you want your presentation to appear casual, choose a font that gives off a similar feeling.

14. Use contrast in your presentation.

Check for contrast between your texts and presentation background to ensure readability. Make it a point to distinguish one from the other.

It’s also worth noting that you are going to show your presentation to a group of people. Depending on the seating arrangement, viewers at the back may find it hard to read your presentation. Make sure that your fonts are of the appropriate size. That way, none of your audience members will have to struggle reading your slides.

15. Consider gifs and memes

Gifs and memes are popular media tools for a good reason. You could incorporate them into your presentation, and they could add a sense of humor to your topic or pitch.

When using gifs and memes, avoid those that could be misinterpreted as politically incorrect or culturally insensitive.

16. Create a consistent look and feel in your slides.

Choose a theme for your presentation templates, and stick with it ’til the end.

17. Ask intriguing questions.

Asking intriguing questions enables you to draw your audience’s attention and highlight key points at the same time.

For example, you are conducting a presentation on the Roman empire. You want to get your audience’s attention, so you raise questions such as what they know about the Roman empire, and how did the Roman empire impact modern society?

The audience may or may not get the right answers, but they will most likely try their best to answer your questions. The resulting exchange of ideas will make your presentation more spontaneous and engaging.

18. Limit to one visual per slide.

Using too many visuals at once will make your presentation appear cluttered. Limit to one visual per slide to help your audience engage more with your text and information.

19. Embrace white space.

White space , also known as negative space, is the space between the lines of texts and visuals in your presentation.  It doesn’t have to be  white  as it can also take the color of your presentation’s background. Think of white space as “empty space”. 

It helps improves readability and ensures that your graphics and texts are clear and legible in your presentation. 

20. End your presentation with an excellent call-to-action.

Call-to-action statements are an integral part of any presentation. They compel your audience to take action, and it makes your presentation more interactive.

Explain what it means to tailor a speech to the audience and the event.

You know that you have to give a presentation and you know there is an audience; what’s next? A bit of research is in order at this point. The more you know about who is listening, the better you will be able to connect your message with them.

Knowing about the audience and the event allows the speaker to tailor the speech for a particular speaking situation. Consider these two scenarios:

Scenario 1 : Janelle, who just completed a summer internship researching PTSD and substance abuse, is invited to present her work to a gathering of physicians. This presentation seems like a perfect opportunity to help medical professionals better understand this complex problem. She puts together a powerful presentation that uses stories from her research to illustrate the vicious circle of trauma and addiction and plans breakout groups in which the physicians would explore how their own work could be improved by a trauma-informed approach to medical care.

Janelle didn’t realize that she’d be speaking at a holiday luncheon.

When she arrives at the event, she discovers that she is one of four students being featured at the event as the recipients of “Rising Research Stars” scholarships. The event itself, she learns, is a holiday luncheon for a local physicians’ group. Even though the scholarship presentation is only a small part of the event, she is allowed to give her full presentation. The crowd seems engaged at first, but she can tell they are losing focus halfway through. 

Afterwards, one of the organizers says, “that’s very impressive work you’re doing. Kind of a downer for a holiday party, though.” He laughs, but Janelle realizes he’s right: she gave the right presentation for the wrong crowd.

In a way, Janelle isn’t the only one at fault here: the organizers didn’t give her enough information to understand her role in the event. The mismatch between her presentation and the audience’s expectations could have been avoided, though, if she had asked a few questions about the nature of the event: What kind of gathering is it? Who will be there? What are they hoping to learn from me?

Knowing about your audience and the event can help to make a presentation successful.

Scenario 2: Jaden needs to train day care employees on changes in their health and safety procedures. Jaden knows that this topic is critical as the workers have to make sure that all the little ones get the best care. Jaden does his homework and asks the day care manager about the attendees—how many there will be along with some demographic information. Even better, Jaden visits the day care to observe the people who will attend.

This prior knowledge gives Jaden information on how to construct his presentation. He will know what type of people will attend, what the facility looks like in which he will speak, age, gender, number, and so on. When Jaden creates his training, he will keep this information in mind. He  can tailor the language, examples, humor, and experience to the group. When Jaden conducts the training, the audience can tell that this training is just for them.

During the speech, Jaden can gauge the audience’s reaction. He can look for visual cues such as eye contact, body language, questions, and so on. He can tell if he is losing his audience if he sees someone dozing off in the back. He can tell if he is winning the audience by head shakes. Jaden will know from this feedback how engaged the audience is and can adjust his presentation based on what he observes.

The final step to knowing how the presentation went is asking for feedback on a survey. You have all experienced surveys given out at the end of speeches. The purpose is to help the speaker improve. Jaden will know from the surveys what his audience thought. He can then incorporate changes or keep some elements based upon what he learns. This is research into how everything went – thus, post feedback.

Tailoring a speech to one’s audience is the best way to get them to be invested in what you’re saying.

Watch out, though! If you tailor your speech too much to one segment of the audience, you risk losing the rest. If you speak just to the experts, your material could sail over the heads of beginners. If you speak only to those who agree with one side of an issue, those who hold the opposing view will dismiss your argument or get angry. Even if your entire audience might agree with your views on something, you should avoid telling them only what they want or expect to hear. This kind of one-sided presentation is called  pandering  or “preaching to the choir.”

Comedian Steve Martin plays with the idea of pandering to the audience on his 1977 standup album  Let’s Get Small . After setting up the idea that a group of plumbers from a convention are at his show, he tells a joke just for the plumbers.

When the joke doesn’t bring down the house, Martin asks, “were those plumbers supposed to be here this show?”

How to prepare and deliver an effective oral presentation

The success of an oral presentation lies in the speaker’s ability to transmit information to the audience. Lucia Hartigan and colleagues describe what they have learnt about delivering an effective scientific oral presentation from their own experiences, and their mistakes

The objective of an oral presentation is to portray large amounts of often complex information in a clear, bite sized fashion. Although some of the success lies in the content, the rest lies in the speaker’s skills in transmitting the information to the audience. 1

Preparation

It is important to be as well prepared as possible. Look at the venue in person, and find out the time allowed for your presentation and for questions, and the size of the audience and their backgrounds, which will allow the presentation to be pitched at the appropriate level.

See what the ambience and temperature are like and check that the format of your presentation is compatible with the available computer. This is particularly important when embedding videos. Before you begin, look at the video on stand-by and make sure the lights are dimmed and the speakers are functioning.

For visual aids, Microsoft PowerPoint or Apple Mac Keynote programmes are usual, although Prezi is increasing in popularity. Save the presentation on a USB stick, with email or cloud storage backup to avoid last minute disasters.

When preparing the presentation, start with an opening slide containing the title of the study, your name, and the date. Begin by addressing and thanking the audience and the organisation that has invited you to speak. Typically, the format includes background, study aims, methodology, results, strengths and weaknesses of the study, and conclusions.

If the study takes a lecturing format, consider including “any questions?” on a slide before you conclude, which will allow the audience to remember the take home messages. Ideally, the audience should remember three of the main points from the presentation. 2

Have a maximum of four short points per slide. If you can display something as a diagram, video, or a graph, use this instead of text and talk around it.

Animation is available in both Microsoft PowerPoint and the Apple Mac Keynote programme, and its use in presentations has been demonstrated to assist in the retention and recall of facts. 3 Do not overuse it, though, as it could make you appear unprofessional. If you show a video or diagram don’t just sit back—use a laser pointer to explain what is happening.

Rehearse your presentation in front of at least one person. Request feedback and amend accordingly. If possible, practise in the venue itself so things will not be unfamiliar on the day. If you appear comfortable, the audience will feel comfortable. Ask colleagues and seniors what questions they would ask and prepare responses to these questions.

It is important to dress appropriately, stand up straight, and project your voice towards the back of the room. Practise using a microphone, or any other presentation aids, in advance. If you don’t have your own presenting style, think of the style of inspirational scientific speakers you have seen and imitate it.

Try to present slides at the rate of around one slide a minute. If you talk too much, you will lose your audience’s attention. The slides or videos should be an adjunct to your presentation, so do not hide behind them, and be proud of the work you are presenting. You should avoid reading the wording on the slides, but instead talk around the content on them.

Maintain eye contact with the audience and remember to smile and pause after each comment, giving your nerves time to settle. Speak slowly and concisely, highlighting key points.

Do not assume that the audience is completely familiar with the topic you are passionate about, but don’t patronise them either. Use every presentation as an opportunity to teach, even your seniors. The information you are presenting may be new to them, but it is always important to know your audience’s background. You can then ensure you do not patronise world experts.

To maintain the audience’s attention, vary the tone and inflection of your voice. If appropriate, use humour, though you should run any comments or jokes past others beforehand and make sure they are culturally appropriate. Check every now and again that the audience is following and offer them the opportunity to ask questions.

Finishing up is the most important part, as this is when you send your take home message with the audience. Slow down, even though time is important at this stage. Conclude with the three key points from the study and leave the slide up for a further few seconds. Do not ramble on. Give the audience a chance to digest the presentation. Conclude by acknowledging those who assisted you in the study, and thank the audience and organisation. If you are presenting in North America, it is usual practice to conclude with an image of the team. If you wish to show references, insert a text box on the appropriate slide with the primary author, year, and paper, although this is not always required.

Answering questions can often feel like the most daunting part, but don’t look upon this as negative. Assume that the audience has listened and is interested in your research. Listen carefully, and if you are unsure about what someone is saying, ask for the question to be rephrased. Thank the audience member for asking the question and keep responses brief and concise. If you are unsure of the answer you can say that the questioner has raised an interesting point that you will have to investigate further. Have someone in the audience who will write down the questions for you, and remember that this is effectively free peer review.

Be proud of your achievements and try to do justice to the work that you and the rest of your group have done. You deserve to be up on that stage, so show off what you have achieved.

Your audience will make up their minds about you in seconds.

How you look and sound during a speech or presentation are going to make a big impression on your audience. Within seconds, listeners will decide whether you are trustworthy, and they will do it based on your body language and vocal attributes. The good news is that there is plenty of hard evidence that explains how you can give the appearance of confidence and competence — even if you’re nervous or timid on the inside. To look confident, make eye contact, keep an open posture, and use gestures to emphasize your message. To sound confident, eliminate filler words, take time to pause before important messages, and vary your pace.

You’ve crafted the message and created the slides for your next presentation. Now it’s time to wow the audience. How you look and sound are going to make a big impression — and your audience will form opinions quickly .

Shelby Co. Clerk Wanda Halbert asks for $2.5 million, delays corrective action plan presentation

Shelby County Clerk Wanda Halbert was set to present a corrective action plan to the Shelby County Board of Commissioners on Wednesday afternoon but deferred her appearance due to a personal matter. The plan is in response to the findings from the Tennessee Comptroller of the Treasury and the county's independent auditors.

Before Halbert spoke to commissioners, two resolutions were passed through the Commission totaling over $2.5 million that were added on late to the agenda. The first add-on, a resolution asking for money to cover additional "essential equipment," like kiosks, scanners and printers, asked for a budget amendment of about $1.8 million.

The money would come from the general fund, and the resolution went down with a favorable recommendation.

The second resolution, another budget amendment, asked for $628,000 to give all clerk's office employees a raise. Director of Human Resources Gerald Thornton told commissioners that the department had no knowledge of the resolution and said it would violate the county's compensation policy.

The resolution went down without a recommendation.

Commissioner Erika Sugarmon presented the two resolutions and garnered one sponsor on one of the amendments from Commissioner Britney Thornton. Both emphasized the double standard that the clerk has been held to and said Halbert has asked numerous times for additional funds and support.

Director of Budget and Fiscal Planning Michael Thompson said that he and the administration have be strong supporters of the clerk and have previously allocated $500,000 to the office for employee raises. Thompson said that the clerk's office did not submit any budget modification requests for fiscal year 2025.

Prior to the committee meeting, the mayor announced that through his budget proposal, all county-wide employees would be receiving a 6% pay raise. Thornton said that it is likely that the increases in the upcoming budget will be better than the proposal Halbert presented, but he could not say if it was for certain because the Clerk did not provide her methodology for the raises.

Multiple times, both Thornton and Thompson reiterated that both their offices had never reviewed the resolutions.

Thompson said any changes to the fiscal year 2024 budget would impact the budget proposal for fiscal year 2025.

Corrective action plan to now be presented May 15

Halbert, the current clerk and former Memphis City Council member, has become a polarizing figure in local politics. From long lines to expired leases on buildings, Halbert has been at the forefront of controversies surrounding the Shelby County Clerk's office.

In late February,  Comptroller Jason E. Mumpower wrote a letter  saying his office would be sending auditors to aid the clerk in submitting revised revenue reports. Days later, Halbert submitted her office's own revised revenue reports  but they were still incorrect . Revenue reports "blessed," by state auditors were  submitted and correct totals are now on the trustee's website .

Mumpower then gave his report on the office after the auditors left. He outlined numerous issues within the office and said his office  "lacks confidence the clerk’s office will be able to accurately report revenue figures in future months."

The corrective action plan resolution, presented by Commissioner Mick Wright, requested the clerk to present a plan to correct the issues found by the auditors. Halbert said that the issues stem from the lack of transition she received from her processor.

"I would never want any other new elected official to walk into this organization and to find it in the condition that we found it in with absolutely no help, little to no help," Halbert said.

Halbert said her office has already begun to address some issues the comptroller found, and she is "grateful" for the help the state auditors provided her.

Halbert was scheduled to present her corrective action plan on May 1st as the resolution requested her to. Since she did not present her corrective action plan May 1st, she will now be requested to appear May 15th.

Spirit house september 4, 2024 – january 26, 2025, the largest of three aaai exhibitions in 2024, this major survey showcases an intergenerational range of contemporary artists—from emerging to established—to foster a transhistorical and transnational dialogue between a diverse group of makers of asian descent.

The Cantor Arts Center at Stanford University is pleased to present  Spirit House , a major group exhibition foregrounding how contemporary artists of the Asian diaspora challenge the boundary between life and death through art. On view from September 4 , 2024 – January 26 , 2025 , the exhibition comprises nearly 50 works of art by 33 artists, including large-scale paintings, digital media, ceramic sculpture, photography, and significant new acquisitions within the museum’s permanent collection. Spirit House is the largest of three major exhibitions in 2024 of the Asian American Art Initiative (AAAI)—a cross-disciplinary, institutional commitment at Stanford University dedicated to the study of artists and makers of Asian descent—curated by AAAI co-founder, Associate Curator Aleesa Pitchamarn Alexander , and will be accompanied by the AAAI’s first publication, an extensive exhibition catalog. Featured artists include: Kelly Akashi , Korakrit Arunanondchai , James Clar , Binh Danh , Dominique Fung , Pao Houa Her , Greg Ito , Tommy Kha , Heesoo Kwon , Timothy Lai , An-My Lê , Dinh Q. I aim to capture and say something about our present moment, and how we might think about art and death to lead a more meaningful life.”  Spirit House asks the question, what does it mean to speak to ghosts, inhabit haunted spaces, be reincarnated, or enter different dimensions? Inspired by spirit houses, small devotional structures found throughout Thailand that provide shelter for the supernatural, this exhibition considers how art can collapse the distance between the past and present, as well as this world and the next. Here, contemporary artists reckon with the spiritual and spectral in our visual culture and question the many forms that ghosts can take. In foregrounding intuitive and inherited forms of knowledge, these artists challenge the primacy of data-driven, scientific methods of understanding the world around us. The exhibition is organized in five thematic sections—“Spirit Houses,” “Ghosts,” “Hauntings,” “Shrines,” and “Dimensions”—that survey how the featured artists are exploring modes of making that exceed rational understanding. Taken together, the works in Spirit House demonstrate how artists have the capacity to act like spirit mediums, materializing prayers and invocations, exorcizing and producing ghosts, and surpassing the limits of linear time through the creative process. These artists bring together family narratives severed by war, migration, and generational trauma, creating new realms and realities. In doing so, they confront the essentially inescapable: life, death, and all that lies in between and beyond.

Spirit Houses “Spirit Houses” explores how architecture and its constituent components have the capacity to hold affect and memory. This section argues for the power of supposedly inanimate spatial forces by emphasizing the primacy of built environments, as exemplified by the work of artists Korakrit Arunanondchai , James Clar , Greg Ito , Maia Cruz Palileo , Namita Paul , Kour Pour , and Do Ho Suh . As the only work of art in the exhibition that directly references (and also transforms) the arrangement of the traditional Thai spirit house, Arunanondchai ’s Shore of Security (2022) is especially significant. Working with the charred pieces of a structure originally created by the artist’s mother, Arunanondchai, like several artists in the exhibition, adopts a collaborative creative process—one that engages directly with a family member—to expand upon childhood memory, rendering an object once deeply familiar to the artist unfamiliar. This work consequently exemplifies the fundamental connection that exists between spirit houses and artworks: both are objects that have the capacity to transcend the limitations of human existence and serve as bridges between realms, as well as mediators of family memory. Ghosts “Ghosts” examines the literal, metaphorical, personal, and collective ghosts that haunt us across time and space. Works in this section chase ghosts—the ancestral lacunae of diasporic life—from the past, present, and future, offering spaces in visual form for intergenerational negotiation and otherwise impossible reunions. Kelly Akashi , Tommy Kha , Timothy Lai , Jarod Lew , Maia Cruz Palileo , Jiab Prachakul , Lien Truong , and Apichatpong Weerasethakul explore the diverse representational and psychological possibilities of incorporating ghosts as narrative devices in their respective practices. For example, throughout her childhood, Truong recalls the haunting presence of the only surviving photograph of her paternal grandmother on her family’s ancestral altar, and reckons with this image and her family’s loss in the painting The Crone (2022), wherein her grandmother’s youthful visage foretells a life cut short during the French occupation of present-day Vietnam.  Hauntings “Hauntings” investigates how historical specters have the capacity to haunt the present and how artists can offer reparative challenges by addressing these conditions. For the artists represented in Spirit House , the historical forces that haunt their works are often directly connected to life circumstances experienced in their ancestral countries of origin, which have contributed to their families’ relocation to the United States. Artists like Binh Danh , Pao Houa Her , An-My Lê , Tidawhitney Lek , and Tuan Andrew Nguyen assert themselves in the wake of the traumatic events that led to their dislocation and transform their inherited histories through reinterpretation and re-presentation. Born in Long Beach, Lek , for example, did not directly experience life under the Khmer Rouge like her family. However, the regime haunts her and manifests in her painting, Refuge (2023) by referencing an autobiography written by the her father about surviving this period—suggesting the continued presence of war within the intimacy of a family’s space, rather than as a discrete event contained with a specific location and chronology.          Other artists in this section include Korakrit Arunanondchai , Dinh Q. Lê , Catalina Ouyang , Masami Teraoka , Salman Toor , and Wanxin Zhang . Shrines The section, “Shrines,” features artists reimagining the iconography of Asian/Asian diasporic shrines to speak to the complexity of family relationships and diasporic culture. Dominique Fung , Reagan Louie , Cathy Lu , Nina Molloy , and Stephanie H. Shih incorporate food items associated with altar offerings in their work to signal that viewers are entering a ritual space, passage, or portal. These works of art summon ancestors, implore the viewer to think past quotidian concerns, and collapse the psychic distance between worlds. In Molloy ’s Shrine (2021–22), for example, oranges appear in front of a portrait of the artist’s great-grandfather, next to a glass of water, and behind a vessel punctuated by incense. The combination of these elements alludes to the holistic nature of ritual offerings: incense stimulates the olfactory sense and summons the deceased, food and drink contributions are gestures of nourishment for the afterlife, and portraits conjure those who have since passed on. In contrast, artists like Shih and Lu also address the social and political dimensions of food offerings, which tell stories about love and care, but also trauma and negotiation.  Dimensions Finally, “Dimensions” shows how human lives and historical events are not isolated but have incarnations in multiple timelines and forms. In this section, artists move within archives and sources to collapse strict chronologies and embody other beings, challenging “the quagmire of the present,” to paraphrase theorist José Esteban Muñoz. Heesoo Kwon , Pao Houa Her , Catalina Ouyang , Kang Seung Lee , Tammy Nguyen , and Amanda Phingbodhipakkiya confront the consequences of human activity in our current era and implore viewers to consider a non-Anthropocene recognition of the world. To do so, these artists contend with the loss of historical figures by considering the radical power of reincarnation. For example, Ouyang ’s love letter to artist and writer Theresa Hak Kyung Cha restages Cha’s iconic 1979 self-portrait to imagine an alternative timeline in which she survives beyond her thirty-one years on earth. Cha was brutally murdered by a security guard at the Puck Building in New York City in 1982, the same year her transformational book Dictee was published. More than four decades later, Ouyang commissioned Larese King, a freelance photographer and security officer at the Puck Building, to take their photograph as a surrogate version of Cha. The resulting picture, presented alongside the artist’s invoice to King, can be read as an honorific gesture from one Asian American artist to another through a suggested rewriting of a historical inevitability.

ABOUT THE ASIAN AMERICAN ART INITIATIVE Based at the Cantor Arts Center, the Asian American Art Initiative (AAAI) is dedicated to the study of artists and makers of Asian descent. Metaplastic breast cancer with a unique presentation and complete response to chemotherapy: a case report

Metaplastic breast carcinomas are a rare variant group of breast carcinomas. They are usually high-grade and triple-negative tumors. They often present with large primary tumor sizes. However, the involvement of axillary lymph nodes is infrequent at the time of diagnosis. Metaplastic breast carcinomas are associated with a worse prognosis and a poorer response to chemotherapy in comparison with other non-metaplastic triple-negative breast cancers. Up until this point, there are no specific treatment recommendations for metaplastic breast carcinomas beyond those intended for invasive breast cancer in general.

Case presentation

A 40-year-old woman complained of a palpable mass in her left axilla. On ultrasonography, the mass was solid, spindle-shaped, hypoechoic with regular borders, and exhibited decreased vascularity. At first, the mass appeared to be of a muscular origin. There was not any clinical nor ultrasonic evidence of a primary breast tumor. On magnetic resonance imaging, the axillary mass was a well-defined with regular borders, measuring 24 × 35 mm. Needle biopsy showed a spindle cell tumor with mild to moderate atypia. The subsequent surgical resection revealed a spindle cell neoplasm within a lymph node, favoring a metastatic origin of the tumor. The tumor cells lacked expression of estrogen, progesterone, and HER2 receptors. PET-CT scan indicated pathological uptake in the left breast. Accordingly, the patient was diagnosed with metaplastic breast cancer that had metastasized to the axillary lymph node. She commenced a combined chemotherapy regimen of doxorubicin and cyclophosphamide. After six treatment cycles, she underwent left modified radical mastectomy with axillary lymph node dissection. Pathological examination of the specimens revealed a total burn-out tumor in the breast due to excellent treatment response. There were no residual tumor cells. All dissected lymph nodes were free of tumor. At the one-year follow-up, the patient showed no signs of tumor recurrence.

This report sheds light on a distinctive presentation of metaplastic breast carcinoma, emphasizing the need for vigilance in diagnosing this rare and aggressive breast cancer variant. In addition, the patient’s remarkable response to chemotherapy highlights potential treatment avenues for metaplastic breast cancer.

Metaplastic breast carcinoma (MpBC) makes up an estimated 0.2–5% of all invasive breast cancers. It represents a heterogeneous group of tumors in which neoplastic cells display differentiation towards squamous epithelium as well as mesenchymal components, such as spindle, chondroid, osseous, or rhabdoid cells [ 1 ].

The WHO classification of metaplastic breast cancer subtypes includes mixed metaplastic carcinoma, low-grade adenosquamous carcinoma, fibromatosis-like, squamous cell carcinoma, spindle cell carcinoma, myoepithelial carcinoma, and metaplastic carcinoma with mesenchymal differentiation (chondroid, osseous) [ 1 ].

MpBCs are often aggressive in nature, typically presenting as high-grade tumors with a triple-negative phenotype (negative for estrogen receptor, progesterone receptor, and HER2) [ 2 ].

Upon presentation, most MpBC patients tend to exhibit large primary tumor sizes, often exceeding 5 cm. Nevertheless, the involvement of axillary lymph nodes is infrequent at the time of diagnosis compared to other types of triple-negative breast cancer (TNBC) [ 3 ].

Considering that MpBC possesses a worse prognosis than other non-metaplastic TNBCs, carries twice the risk of recurrence, and has a shorter disease-free and overall survival (71% 5-year overall survival for MpBC compared to 88% for IDC) [ 4 ], it is worth noting that The National Comprehensive Cancer Network (NCCN) guidelines do not offer specific treatment recommendations for MpBC beyond those intended for invasive breast carcinoma in general [ 5 ].

Over the past years, conventional therapies including surgery, chemotherapy, and radiation have been used to treat MpBC. However, the inadequacy of these treatment methods, as reflected by poor survival rates, has necessitated the need for novel therapeutic options such as targeted and immunotherapies, which are still undergoing experimental trials and evaluation [ 6 ].

The objective of this study was to report a case of metaplastic breast cancer with a unique presentation and complete response to chemotherapy.

A 40-year-old woman presented to the clinic complaining of a palpable mass in her left axilla, which had been present for two months. Otherwise, she had no other complaints. Her medical history was unremarkable.

Upon physical examination, the left axillary mass was fixed and measured approximately 30 mm in size, displaying no visible signs on the skin such as redness, ulceration, or swelling. Additionally, there was nipple induration noted in both breasts in the absence of any palpable masses.

Ultrasonography revealed numerous benign cysts in both the left and right breasts. In addition, it detected two fibroadenomas in the left breast: one measuring 13–20 mm and the other, smaller at 12 mm but notably hypoechoic. The left axillary mass was solid, spindle-shaped, hypoechoic with regular borders, and exhibited decreased vascularity (Fig.  1 ). It measured approximately 31 mm. It was adjacent to the axillary artery and contiguous with the tendon of one of the rotator cuff muscles. Thus far, the mass appeared to be of a muscular origin. Moreover, ultrasonography identified three enlarged lymph nodes that were hypoechoic and measuring 7–8 mm.

Mammography did not reveal any additional findings beyond those obtained by ultrasound.

Magnetic resonance imaging (MRI) of the left axilla demonstrated a well-defined mass with regular borders, measuring 24 × 35 mm. It was directly adjacent to the axillary nerve that they were practically indistinguishable (Fig.  2 ). On T1-weighted images, the lesion exhibited low signal intensity, while on T2-weighted images, it demonstrated heterogeneously high signal intensity. Following contrast administration, the mass showed heterogeneously marked enhancement.

In accordance with MRI findings, the differential diagnosis primarily included nerve tumors such as schwannoma and possibly a metastatic lymph node.

A needle biopsy of the axillary mass showed a spindle cell tumor with mild to moderate atypia. An excisional biopsy was recommended for comprehensive evaluation.

The subsequent mass resection pathology revealed a malignant high-grade spindle cell neoplasm within a lymph node, favoring a metastatic origin of the tumor. Immunostaining of the sample was positive for CK (Cytokeratin), Vimentin (focally), and S100 markers and negative for TLE (Transducin-like enhancer of split), CD34, CK5/6, and p63 markers. Furthermore, the tumor cells lacked expression of estrogen, progesterone, and HER2 receptors (triple negative). The Ki-67 index was low (< 10%).

PET-CT scan indicated pathological uptake of FDG in the upper inner quadrant of the left breast (SUV = 3) (Fig.  3 ). There was no pathological metabolic activity observed in other parts of the body.

Accordingly, the diagnosis was triple-negative metaplastic breast cancer (of a spindle cell carcinoma subtype) that had metastasized to the axillary lymph node.

The patient subsequently commenced a combined chemotherapy regimen of doxorubicin (Adriamycin) and cyclophosphamide (AC).

After completing six treatment cycles, there was a mild decrease in nipple induration and the patient underwent left modified radical mastectomy with axillary lymph node dissection as the accurate primary tumor location could not be identified because no biopsy was taken from the primary tumor and no clip was placed in its site, as this technique is not available in Syria anyway [ 7 , 8 ].

Pathological examination of the surgical specimens revealed a total burn-out tumor in the breast due to excellent treatment response. There were no residual tumor cells. All 19 dissected lymph nodes were reactive and free of tumor.

At the one-year follow-up, the patient showed no signs of tumor recurrence.

Discussion and conclusions

Metaplastic breast carcinoma (MpBC) is a rare subtype of invasive breast cancer characterized by the presence of both epithelial and mesenchymal elements and typically presents as an aggressive triple-negative tumor [ 1 , 2 ].

MpBC usually manifests as a palpable breast mass [ 9 ]. However, it often spares axillary lymph nodes. In an analysis of 892 MpBC cases, only 174 (21.9%) had axillary node involvement at presentation [ 2 ]. Even in studies with smaller samples, the percentage of patients with positive axillary lymph nodes at diagnosis did not exceed 38–40% [ 10 , 11 ].

In contrast, our patient presented uniquely with a relatively big axillary mass, which was later identified as a metastatic lymph node in the absence of any clinical evidence of the primary breast tumor.

Ultrasonography of MpBCs usually shows irregular tumor shadow, micro-lobulated margins, complex echogenicity, parallel orientation, and posterior acoustic enhancement [ 9 ]. On the other hand, the most common feature of MpBCs on MRI is the high intensity of signal on T2-weighted images [ 12 ]. In addition, these tumors usually have irregular margins and display heterogeneous internal enhancement on MRI [ 9 ].

In our case, ultrasonography and mammography of the breasts were only remarkable for multiple bilateral cysts and two benign lesions in the left breast, which were considered fibroadenomas at that time. They did not reveal any suspicious findings, which made the breast origin of the axillary mass less likely at first.

However, MpBCs tend to display more benign features, such as round or oval shapes with well-defined borders, in comparison with ductal carcinomas [ 13 ]. The circumscribed margins of the mass on mammography might reflect the spindle-cell component of these tumors [ 14 ].

Relative to other non-metaplastic TNBCs, MpBCs have a worse prognosis and poorer response to chemotherapy. While TNBCs typically exhibit a pathological complete response (pCR) rate of 33.6% to neoadjuvant chemotherapy [ 15 ], the response rates for MpBCs are generally lower. Results from four studies on neoadjuvant chemotherapy for MpBC indicated pCR rates of 2% in one study involving 44 patients [ 16 ], 17% in another study with 29 patients [ 17 ], 11% in a study with 18 patients [ 18 ], and 16% in the last one involving six patients [ 19 ].

Despite the unique presentation, our patient achieved a pCR to the combined chemotherapy regimen (AC), highlighting its potential role in unusual cases of MpBC.

In the upcoming era, chemotherapy alone might not constitute the definitive curative approach for MpBC. However, it could be used in combination with targeted and immunotherapies, which are still undergoing experimental trials and evaluation [ 6 ].

Novel therapeutic options for MpBC are emerging based on the specific molecular characteristics of the disease. TP53 mutations, which are very common in MpBC, and mutations in PI3K have been identified as promising targets for therapy [ 20 , 21 , 22 ].

TP53 mutations are also associated with elevated VEGF-A levels, suggesting potential sensitivity to anti-VEGF agents such as bevacizumab [ 23 , 24 ]. Notably, mTOR inhibitors such as temsirolimus and everolimus have shown secondary effects on angiogenesis, providing the base for investigating combinations with bevacizumab in clinical trials [ 25 , 26 ]. Furthermore, the combination of temsirolimus with bevacizumab and various chemotherapy agents, including platinums, taxanes, and anthracyclines, has been studied [ 27 ].

Additionally, MpBC is characterized by the overexpression of PD-L1 and high tumor-infiltrating lymphocytes (TIL), which set the basis for treating MpBC with immunotherapeutic agents [ 22 , 28 ].

In summary, this report sheds light on a distinctive presentation of metaplastic breast carcinoma (MpBC), emphasizing the need for vigilance in diagnosing this rare and aggressive breast cancer variant. In addition, the patient’s remarkable response to chemotherapy highlights potential treatment avenues for MpBC.

One limitation of our study was the absence of suspicious findings on breast imaging, which precluded the opportunity to perform a biopsy for confirmation. Consequently, the diagnosis relied on the correlation between PET-CT scan findings and lymph node pathology.

Another limitation to acknowledge is that our study was conducted on a single patient. While this case provided valuable insights, the findings may not be fully generalizable to a broader population and larger studies are required.

Opinion: Salt Lake City School District’s restroom presentation harms students. As nonbinary and transgender educators, we see the damage.

The directive to give presentations in every single classroom created widespread harm and put educators in precarious positions to balance their “professionalism” with their humanity..

Utah will become the latest state to implement restrictions for transgender people using school bathrooms and locker rooms in public schools and government-owned buildings when key components of a law passed by the Republican controlled Legislature take effect May 1.

On May 1, HB 257 went into effect in K-12 public schools. But for us, transgender and nonbinary educators in the Salt Lake City School District, the effect has been felt for weeks. We feel compelled to speak as private citizens, not as representatives of the district or the schools we work in, due to the impact on our queer and transgender community in Salt Lake City.

We chose to work in this district specifically because of its commitment to equity. Salt Lake City School District (SLCSD) has a vision statement that reads “Excellence and Equity: every student, every classroom, every day” in addition to being the first “ Dignity District ” nationwide, with the commitment “to learning and work environments where everyone is treated with dignity.” Yet, SLCSD’s directive to have teachers give classroom presentations about HB 257 undermines its own commitment to dignity and equity, resulting in direct harm to transgender students, educators and their allies.

SLCSD created presentations for grades K-5 and 6-12 which stated that “new legislation requires us” to tell students they needed to use the bathroom of the gender they were assigned at birth. In the creation of these presentations , SLCSD failed to recognize the impact this would have on teachers, counselors, administrators and psychologists who were left with no support or resources to honor our own dignity. While the district’s recent newsletter stated that it would “remain committed to making sure our schools remain safe, welcoming places for all our students, families, and staff,” we were not provided with adequate means of support to do so.

Instead, we as transgender and nonbinary educators took on the labor to answer questions from coworkers on how to best support our students. With no district acknowledgment of the pain it caused us, transgender and nonbinary educators were also expected to give this presentation to our students, directly harming them and betraying our community when we know exactly what the implications are — 56% of transgender and nonbinary youth in Utah seriously considered suicide in 2022 . The district did not recognize this potential for harm in any communications to faculty, staff or families.

We took it upon ourselves to create our own presentations filled with local and national resources, support systems and hotlines, crafted for both secondary and elementary communities — the resources to help fellow educators support impacted students and families that our district should have created. We shared these with the educators we could within our reach. Subsequently, we watched as some of our students’ fiercest allies and our schools’ kindest hearts experienced the chilling effect of the presentation: Pride flags, safe school posters and pronoun pins were fearfully stowed away. We continue to question: How does this make our district safe and welcoming?

In secondary schools, regardless of their gender identity, students questioned why this was the action the district decided to take when their friends in other districts were not subject to the same trauma and harm. Several stated they would rather the district make a presentation about vaping, drug use and truancy in the bathrooms instead — and stop targeting their trans and nonbinary peers. At the elementary level, young trans and nonbinary students already struggle to get peers and teachers to acknowledge and understand their identities. Many aren’t even “out,” choosing to be accepted simply for who they are without a label that could subject them to interrogation. Coupling this reality with the introduction of HB 257 means we have students of all ages, K-12, who had their right to use the bathroom become subject to classroom discussion.

We recognize that SLCSD needs to ensure they comply with state law and the specific directives of HB 257. However, the directive to give presentations in every single classroom created widespread harm and put educators in precarious positions to balance their “professionalism” with their humanity. SLCSD did not provide justification for choosing to present the presentations over the methods other districts in our state used to notify students of HB 257.

We call for SLCSD to recognize the impact that it had on transgender, nonbinary and allied educators, students, staff and administrators. The effects of this erasure by the district were unnecessary, undignified and dehumanizing. The presentations did not give us reason to believe our district’s stated values and vision statement.

Additionally, we need actionable, tangible changes to create safe, equitable and dignified environments for everyone who works and learns in SLCSD. We ask the district to contract a professional development session on LGBTQIA+ inclusion, preferably through a local organization. We also ask that SLCSD conduct a thorough audit of their policies in the wake of new federal Title IX guidance which states that sex-separated programs and activities, including bathrooms, cannot exclude a person from a space consistent with their gender identity.

We have seen displays of love and support for transgender and nonbinary students and staff both in and out of our buildings and recognize its positive impact. We hope that SCLSD can join in these displays and that these actions will hopefully ensure that our district’s transgender and nonbinary community never feels this way again.

Porn actor Stormy Daniels is taking the witness stand in Donald Trump’s New York hush money trial. Trump faces 34 felony counts in connection to payments meant to stifle negative stories from surfacing in the final days of the 2016 presidential campaign.

NEW YORK (AP) — Porn actor Stormy Daniels took the witness stand Tuesday in the hush money case against Donald Trump, who looked on as she detailed their alleged sexual encounter and the payment she got to keep it quiet.

Prosecutors allege Trump paid Daniels to keep quiet about the claims as he ran for president in 2016. Her testimony aired them very publicly as the presumptive Republican presidential nominee seeks to win the White House again.

Trump denies having sex with Daniels, and his lawyers unsuccessfully pushed for a mistrial midway through her testimony.

It’s the biggest spectacle yet in the first criminal trial of a former American president, now in its third week of testimony in Manhattan.

Here are some takeaways from Daniels’ testimony:

WHO IS STORMY DANIELS?

The case centers on a $130,000 payment to Daniels from Trump’s then-lawyer, Michael Cohen, in the final weeks of Trump’s 2016 campaign. Prosecutors say it was part of a scheme to illegally influence the campaign by burying negative stories about him.

His lawyers have sought to show that Trump was trying to protect his reputation and family — not his campaign — by shielding them from embarrassing stories about his personal life.

Daniels, whose real name is Stephanie Clifford , told jurors that she started exotic dancing in high school and appearing in adult films at age 23, eventually moving on to direct more than 150 films and winning a roster of porn industry awards.

MEETING TRUMP

Daniels testified she first met and chatted with Trump at a 2006 Lake Tahoe celebrity golf outing where her studio was a sponsor.

He referred to her as “the smart one” and asked her if she wanted to go to dinner, she said. Daniels testified that she accepted Trump’s invitation because she wanted to avoid dinner with her co-workers and thought it might help her career. Trump had his bodyguard get her number, she said.

When they met up later in his penthouse, she appreciated that he seemed interested in the business aspects of the industry rather than the “sexy stuff.” He also suggested putting her on his TV show, “The Apprentice,” a possibility she hoped could help establish her as a writer and director.

She left to use the bathroom and was startled to find Trump in his underwear when she returned, she said. She didn’t feel physically or verbally threatened but realized that he was “bigger and blocking the way,” she testified.

“The next thing I know was: I was on the bed,” and they were having sex, Daniels recalled. The encounter was brief but left her “shaking,” she said. “I just wanted to leave,” she testified.

PAYMENTS FOR SILENCE

Daniels was asked if Trump ever told her to keep things between them confidential and said, “Absolutely not.” She said she learned in 2011 that a magazine had learned the story of their encounter and she agreed to do an interview for $15,000 to make money and “control the narrative.” The story never ran.

In 2016, when Trump was running for president, Daniels said she authorized her manager to shop the story around but did not initially receive interest from news outlets. She said that changed in October with the release of the “Access Hollywood” tape in which Trump bragged about grabbing women sexually without asking permission. She said she learned that Cohen wanted to buy her silence.

MISTRIAL PUSH

Midway through her testimony, Trump’s lawyers moved for a mistrial.

Defense lawyer Todd Blanche argued that Daniels’ testimony about the alleged encounter and other meetings with him had “nothing to do with this case,” and would unfairly prejudice the jury.

The judge rejected it, and he faulted defense attorneys for not raising more of their objections while she was testifying.

Before Daniels took the stand, Trump’s lawyers had tried to stop her from testifying about the encounter’s details, saying it was irrelevant in “a case about books and records.”

Prosecutors countered that Daniels’ testimony gets at what Trump was trying to hide and they were “very mindful” not to draw too much graphic detail. Before Daniels took the stand, they told the judge the testimony would be “really basic,” and would not “involve any details of genitalia.”

While the judge didn’t side with Trump’s lawyers, he acknowledged that some details were excessive. The objections could potentially be used by Trump’s lawyers if he is convicted and they file an appeal.

CROSS-EXAMINATION

Trump’s lawyers tried to attack Daniels’ credibility, suggesting she was motivated by money and that her account has shifted over the years.

“Am I correct that you hate President Trump?” Defense lawyer Susan Necheles asked Daniels at one point. Daniels acknowledged she did.

“And you want him to go to jail?” the lawyer asked.

“I want him to be held accountable,” Daniels said. Pressed again whether that meant going to jail, she said, “If he’s convicted.”

The defense pressed Daniels on the fact that she owes Trump hundreds of thousands of dollars in legal fees stemming from an unsuccessful defamation lawsuit and on a 2022 tweet in which she said she “will go to jail before I pay a penny.” Daniels dug in at times in the face of pointed questions, forcefully denying the idea that she had tried to extort Trump.

TRUMP IN COURT

Trump whispered frequently to his attorney during Daniels’ testimony, and his expression seemed to be pained at one point as she recounted details about the dinner she says they shared. He shook his head and appeared to say something under his breath as Daniels testified that Trump told her he didn’t sleep in the same room as his wife.

At one point, the judge told defense lawyers during a sidebar conversation — out of earshot of the jury and the public — that he could hear Trump “cursing audibly” and see him shaking his head, according to a transcript of the proceedings.

“I am speaking to you here at the bench because I don’t want to embarrass him,” Judge Juan Merchan told Blanche, Trump’s lawyer. Blanche assured the judge he would talk to his client.

“You need to speak to him. I won’t tolerate that,” the judge said.

On the way out of the courthouse, Trump called it “a very revealing day.” He didn’t address Daniels’ testimony explicitly but claimed the prosecutors’ case was “totally falling apart.”

A JARRING SPLIT SCREEN

Trump’s appearance in court Tuesday, like all other days he’s stuck in the courtroom, means he can’t be out on the campaign trail as he runs for president a third time. It’s a frequent source of his complaints, but Daniels’ testimony in particular might underscore how much of a distraction the trial is from the business of running for president.

While Trump was stuck in a Manhattan courthouse away from voters and unable to speak for much of the day, his rival, Democratic President Joe Biden, was attending a Holocaust remembrance ceremony and condemning antisemitism.

It’s an issue Trump has sought to use against Biden in the campaign by seizing on the protests at college campuses over the Israel-Hamas war.
