Whether you are practicing evidence-informed decision making or conducting research (e.g. a systematic review), the first step of the process is to clearly define the question or problem in a searchable and answerable format. This step of the process helps you answer the questions:

“Who is my target group?
What is the issue we are dealing with?
What specifically are we trying to change?”

Using a framework to develop your question can help to identify the main concepts of your review topic (see further down the page). Whether you use a framework or not, the goal is to identify the main concepts of your question and the synonyms / similar terms that might be used to describe each of those concepts:

Combining search terms with AND or OR:

Use AND to combine different main concepts of your search: childhood AND obesity. Narrows results
Use OR to include similar terms / synonyms (and sometimes antonyms) for a concept: childhood OR adolescence. Fertility OR infertility. Broadens results

Considerations for searching:

It is not always necessary to include all of your main concepts in the search. In some PICO questions, the outcome (O) is implied and does not need to be included. In other cases, any and all outcomes might of interest and so the search strategy can leave this out to keep it open.

Example for the research question: In seniors with dementia, does a falls prevention program, compared to no falls prevention program, result in decreased falls?

In the following PICO question, the comparator (C) can be left out of the search strategy as there is no great way to search for "not" having the intervention (in contrast to when there is a placebo or an alternate intervention as a comparison)
Also, the intervention (I) somewhat implies the outcome (O), so the outcome (decreased falls) can be left out of the search strategy
The main concepts that should be included in this particular search are: seniors (P1), dementia (P2), and falls prevention (I). Keep in mind though that there are different ways of describing these three concepts and so synonyms / similar terms are important to include in the search as well

Applying a framework when developing a question can help to identify the key concepts and determine inclusion and exclusion criteria.

How geography acts as a structural determinant of health

In unincorporated communities in the United States-Mexico borderlands, historically and socially marginalized populations become invisible to the healthcare system, showing that geography acts as a structural determinant of health for low-income populations. So concludes a study by a University of California, Riverside, team that focused its attention on the borderland in Southern California, specifically, eastern Coachella Valley.

From September to December 2020, the team, led by Ann Cheney, an associate professor of social medicine, population, and public health in the School of Medicine, conducted interviews in collaboration with María Pozar, a community investigator and CEO of Conchita Servicios de la Comunidad, with 36 Latinx and Indigenous Mexican caregivers of children with asthma or respiratory distress. The researchers found communities in the "colonias" (unincorporated areas in the borderlands) lack basic critical infrastructure including healthcare access.

The U.S.-Mexico borderland is home to nearly 2.7 million Hispanic or Latinx individuals. The immigrant population in the colonias has limited English proficiency, health literacy levels, and income, and lower levels of formal education. Many are undocumented.

"Our work shows the importance of geography in health and how geography acts as a structural determinant of health," Cheney said. "For example, foreign-born caregivers who speak Spanish or Purépecha prefer to take their children across the U.S.-Mexico border for respiratory health care because physicians there provide them with a diagnosis and treatment plan that they perceive improves their children's health."

The study, published in the journal Social Science & Medicine , found the caregivers perceive U.S.-based physicians as not providing them with sufficient information since most physicians do not speak their language and do not adequately listen to or are dismissive of their concerns about their children's respiratory health. The caregivers perceive Mexican-based physicians as providing them with a diagnosis and treatment plan, whereas U.S.-based physicians often prescribe medications and provide no concrete diagnosis.

"Further, only those with legal documentation status can cross the border, which contributes to disparities in children's respiratory health," Cheney said. "Thus, caregivers without legal status in the U.S. must access healthcare services in the U.S. for their children and receive, what these caregivers perceive, as suboptimal care."

Cheney added she was surprised to learn that caregivers who did not have legal documentation status in the U.S. asked trusted family and friends to take their children across the border to receive healthcare services for childhood asthma and related conditions.

"Geography, meaning living in unincorporated communities, harms health," she said. "Geography and the politics of place determines who can and cannot cross borders."

Study participants discussed the distance they needed to travel to pediatric specialty care for the care and management of their children's respiratory health problems. Some commented on the lack of interaction and communication with physicians during medical visits. Some participants commented on the lack of physicians' knowledge about the connections between their children's exposure to environmental hazards and poor respiratory health and allergic symptoms.

The research took place in four unincorporated rural communities -- Mecca, Oasis, Thermal, and North Shore -- in eastern Coachella Valley, along the northern section of the Salton Sea. People living in the colonias here are subject to the health effects of environmental hazards. Many are farmworkers living and working in the nearby agricultural fields. Most of the workforce lives in mobile parks and below the federal poverty line.

"In addition to toxic water and dust from the Salton Sea, other environmental health hazards, such as agriculture pesticide exposure, waste processing facilities, and unauthorized waste dumps, also contribute to this community's high incidence of poor respiratory health," said Gabriela Ortiz, the first author of the research paper and a graduate student in anthropology who works with Cheney. "These communities are vulnerable to the policies and governing decisions around exposure to environmental hazards and infrastructure development. The absence of infrastructure and lack of healthcare infrastructure limits their access to primary care and specialty care services."

Ortiz explained that anthropologists and social scientists have long argued that environmental injustices are a product of structural violence.

"This is indirect violence caused by social structures and institutions that prevent individuals from meeting their basic needs because of political economic domination and class-based exploitation," she said. "Understanding the complex interplay between geography, borderlands, and health is essential for coming up with effective public health policy and interventions."

The title of the research paper is "Seeking care across the US-Mexico border: The experiences of Latinx and Indigenous Mexican caregivers of children with asthma or respiratory distress."

Cheney, Ortiz, and Pozar were joined in the study by Ashley Moran and Sophia Rodriquez of UCR.

The study was funded by the National Institutes of Health/National Institute of Minority Health and Health Disparities.

Conference Facilitates Talk on Hot Topics in Health Policy and Management

From artificial intelligence to alternative medicine and quality care to communications: the annual Health Policy and Management (HPM) Healthcare Conference was a sounding board for ideas on all manner of timely issues affecting organizational leaders, policymakers, and not least, everyone at the receiving end of health care services. The all-day event on April 12 brought together faculty, students, policymakers, and industry leaders. The event was sponsored by The Commonwealth Fund and held at Columbia University event space The Forum.

Sherry Glied, Dean of New York University’s Robert F. Wagner Graduate School of Public Service and former HPM chair, opened the conference with a keynote address on the future of healthcare in New York State and beyond. Glied, who chairs the New York State Commission of the Future of Healthcare, said her goal was to recommit the state to measuring success through “the health and well-being of individual people.”

A morning panel titled Communicating Topics in Public Health Effectively shed light on strategies to convey health information to the public despite the challenging political environment. HPM Professor Robert Shepardson moderated the panel, which included White House Assistant Press Secretary Kevin Munoz, Deputy Assistant Secretary for Strategic Planning and Speechwriter for U.S. Department of Health and Human Services Brian Reich, and Environmental Health Sciences Professor Julie Herbstman . When asked why communications is important, Herbstman, who is director of Columbia Center for Children’s Environmental Health noted, “I love working with data, but the reason I’m doing public health research is so that it goes somewhere, it has some kind of impact. My job isn’t done when I publish a paper.”

Afternoon sessions covered topics including food justice, developments in alternative medicine, PEPFAR reauthorization, and a lunch salon focused on digital health. The latter discussion was led by HPM student Andera Marx, senior marketing manager at Amazon Health Services and senior product marketing manager at One Medical.

The closing panel considered the deployment of AI in healthcare and what that could mean for equitable access to services. Columbia Mailman Biostatistics Professor and Associate Dean for Data Science Jeff Goldsmith moderated the event, which included panelists Julia Iyasere, director of the Dalio Center of Health Justice; Ashley Beecy, medical director of AI Operations at NewYork-Presbyterian; and physician-strategist Rebecca Winkour.

The day concluded with a networking reception and panel for admitted students led by current master’s students. Amey Kamdar, MHA ’24, appreciated how the conference brought together providers, researchers, policymakers, and students, adding, “That made for great dialogue and broadening of horizons.” HPM Chair and Professor of Health Policy and Management Michael Sparer echoed Kamdar’s comments, saying, “This is a special day where we come together once a year and we hear great talks, but we also have an opportunity to network and be with each other.”

Mental health and the pandemic: What U.S. surveys have found

The coronavirus pandemic has been associated with worsening mental health among people in the United States and around the world . In the U.S, the COVID-19 outbreak in early 2020 caused widespread lockdowns and disruptions in daily life while triggering a short but severe economic recession that resulted in widespread unemployment. Three years later, Americans have largely returned to normal activities, but challenges with mental health remain.

Here’s a look at what surveys by Pew Research Center and other organizations have found about Americans’ mental health during the pandemic. These findings reflect a snapshot in time, and it’s possible that attitudes and experiences may have changed since these surveys were fielded. It’s also important to note that concerns about mental health were common in the U.S. long before the arrival of COVID-19 .

Three years into the COVID-19 outbreak in the United States , Pew Research Center published this collection of survey findings about Americans’ challenges with mental health during the pandemic. All findings are previously published. Methodological information about each survey cited here, including the sample sizes and field dates, can be found by following the links in the text.

The research behind the first item in this analysis, examining Americans’ experiences with psychological distress, benefited from the advice and counsel of the COVID-19 and mental health measurement group at Johns Hopkins Bloomberg School of Public Health.

At least four-in-ten U.S. adults (41%) have experienced high levels of psychological distress at some point during the pandemic, according to four Pew Research Center surveys conducted between March 2020 and September 2022.

Young adults are especially likely to have faced high levels of psychological distress since the COVID-19 outbreak began: 58% of Americans ages 18 to 29 fall into this category, based on their answers in at least one of these four surveys.

Women are much more likely than men to have experienced high psychological distress (48% vs. 32%), as are people in lower-income households (53%) when compared with those in middle-income (38%) or upper-income (30%) households.

In addition, roughly two-thirds (66%) of adults who have a disability or health condition that prevents them from participating fully in work, school, housework or other activities have experienced a high level of distress during the pandemic.

The Center measured Americans’ psychological distress by asking them a series of five questions on subjects including loneliness, anxiety and trouble sleeping in the past week. The questions are not a clinical measure, nor a diagnostic tool. Instead, they describe people’s emotional experiences during the week before being surveyed.

While these questions did not ask specifically about the pandemic, a sixth question did, inquiring whether respondents had “had physical reactions, such as sweating, trouble breathing, nausea, or a pounding heart” when thinking about their experience with the coronavirus outbreak. In September 2022, the most recent time this question was asked, 14% of Americans said they’d experienced this at least some or a little of the time in the past seven days.

More than a third of high school students have reported mental health challenges during the pandemic. In a survey conducted by the Centers for Disease Control and Prevention from January to June 2021, 37% of students at public and private high schools said their mental health was not good most or all of the time during the pandemic. That included roughly half of girls (49%) and about a quarter of boys (24%).

In the same survey, an even larger share of high school students (44%) said that at some point during the previous 12 months, they had felt sad or hopeless almost every day for two or more weeks in a row – to the point where they had stopped doing some usual activities. Roughly six-in-ten high school girls (57%) said this, as did 31% of boys.

On both questions, high school students who identify as lesbian, gay, bisexual, other or questioning were far more likely than heterosexual students to report negative experiences related to their mental health.

Mental health tops the list of worries that U.S. parents express about their kids’ well-being, according to a fall 2022 Pew Research Center survey of parents with children younger than 18. In that survey, four-in-ten U.S. parents said they’re extremely or very worried about their children struggling with anxiety or depression. That was greater than the share of parents who expressed high levels of concern over seven other dangers asked about.

While the fall 2022 survey was fielded amid the coronavirus outbreak, it did not ask about parental worries in the specific context of the pandemic. It’s also important to note that parental concerns about their kids struggling with anxiety and depression were common long before the pandemic, too . (Due to changes in question wording, the results from the fall 2022 survey of parents are not directly comparable with those from an earlier Center survey of parents, conducted in 2015.)

Among parents of teenagers, roughly three-in-ten (28%) are extremely or very worried that their teen’s use of social media could lead to problems with anxiety or depression, according to a spring 2022 survey of parents with children ages 13 to 17 . Parents of teen girls were more likely than parents of teen boys to be extremely or very worried on this front (32% vs. 24%). And Hispanic parents (37%) were more likely than those who are Black or White (26% each) to express a great deal of concern about this. (There were not enough Asian American parents in the sample to analyze separately. This survey also did not ask about parental concerns specifically in the context of the pandemic.)

Looking back, many K-12 parents say the first year of the coronavirus pandemic had a negative effect on their children’s emotional health. In a fall 2022 survey of parents with K-12 children , 48% said the first year of the pandemic had a very or somewhat negative impact on their children’s emotional well-being, while 39% said it had neither a positive nor negative effect. A small share of parents (7%) said the first year of the pandemic had a very or somewhat positive effect in this regard.

White parents and those from upper-income households were especially likely to say the first year of the pandemic had a negative emotional impact on their K-12 children.

While around half of K-12 parents said the first year of the pandemic had a negative emotional impact on their kids, a larger share (61%) said it had a negative effect on their children’s education.
